Learning from Canadians with Chronic Health Conditions

This report continues our focus on the urgent challenge of chronic health conditions, their impact on health care services and on quality of life. We present a case for action to close the gap between what we know and what we do to prevent chronic conditions and to care for people who have them.

We learn from Canadians about their recent experiences with chronic illness care, and we report on activities underway across the country to improve prevention and reorient care.
